A Rule 25 assessment of chemical dependency, conducted by a Hennepin County approved Rule 25 Assessor.

Confidentiality and Contact

Confidentiality:
All files and data on participants are governed by Turning Point's Data Privacy Policy as set by the governing Board of Directors. A participant will have a right to view information in his file only as it pertains to his chemical dependency treatment. No information in a participant's file may be shared with any other person or agency that is not a part of the chemical dependency process without the consent of the participant. All participants are required to file a Privacy Act form to ensure privacy and confidentiality of treatment and services rendered at Turning Point.
