Our standards for Turning Point's Case Management program are:
Assessment
A comprehensive assessment to include health status; mental health and history status; chemical health history and status; sexual health; HIV transmission risk; current medical care provider; health care coverage; knowledge of HIV transmission; social and emotional support; key relationships; financial status; housing; and legal issues.
Access to Medical Care Plan
The goal is to have the client either covered by a medical care plan or actively seeking medical care coverage within 3 months of intake.
Risk Reduction
Involves an assessment and understanding of client's behaviors that transmit the HIV virus. Activities for educating the client about risk reduction are noted in the Service Care Plan.
Advocacy
Turning Point case management staff act as advocates for their clients, with a plan for teaching the client how to be an advocate for him or herself.
Coordination With Medical Care
The goal here is to help clients maximize the efficiency and effectiveness of their medical care plan.
Referrals
Turning Point case management staff knows what services are commonly used by HIV positive clients. Once a referral is made, the referral and its outcome are charted in the client's file.
HIV Disease and Medical Education
Turning Point case managers are a resource for clients to learn about HIV and effective medical treatment.
Client Rights, Level of Service, Confidentiality, Reporting, Grievances and Appeals, and Informed Consent
Case Management staff assures clients prompt and reliable service. Clients are also advised of their rights as a client. Confidentiality of each client is assured. However, in some instances, reporting to a third party is mandatory. For this, clients sign a release form. Case managers are required to know and abide by laws pertaining to Duty to Warn, Vulnerable Adults, and Child Maltreatment. Clients have a right to fair treatment by their Case Manager. Turning Point provides procedures for client grievances and appeals according to guidelines set by Hennepin County and the State of Minnesota. All clients have a right to know what services they are receiving and at what standards.
